Care
Every resident has an individualised care package created for them. This commences prior to admission at a meeting arranged either at The Shelley, your own home, or at hospital. A trained member of staff will take time to get to know you and with the help of family or friends, assess your personal health, social needs and expectations. Individual wishes and choices are taken fully into account and a plan of care is developed for you. You will have full access to your care plan and encouraged to participate as fully as possible in its monthly review.
Each plan sets out the objectives and how we aim to meet them, risk assessments are also incorporated to form part of the care plans. All changes are recorded and reviewed on a monthly basis. Further, we liase with the GPs and other healthcare professionals to address our resident’s individual needs .
Care packages range from minimum assistance to an intensive package of care.
